REGISTRATION 

FAHQ is pleased to offer registration rates used for the past two conferences. Our sponsors/exhibitors are key to keeping registration fees at this level. When you attend our conference make sure you thank our exhibitors and sponsor representatives.

Not yet a member of FAHQ? Consider the Join and Register option. For $40.00 over the member registration fee(s) you will receive a full year of membership in FAHQ, saving you $15.00 over a regular individual membership. (Available for new members only, NOT for renewing members.)

Group Discount: Groups of 5 or more may receive a 10% discount on registration for the Conference and Pre-Conference sessions. Group registration payment must be submitted offline, with all members of the group listed on the same Offline Payment Form. Download Group Form. Before sending payment, EACH group member must register here in the appropriate registration category and select Offline Payment. Registration will be confirmed when payment received.

Group discount DOES NOT apply to the CPHQ Review Course.

Save $50 on each conference component you register for if you register on or before the deadline of April 1, 2018.

Offline payment: Your registration can be processed via mail using your personal check or your employer’s check. Proceed with the on-line registration process, when prompted please print the Offline Registration Form and ensure it is mailed with the payment.

Online payment: Your registration can be processed on-line using your debit card or a personal or corporate credit card.

REFUND POLICY

Refunds will be issued if requested prior to April 1, 2018 but are subject to a $75 processing fee. No refunds issued after that date. Registration may be transferred to a substitute if notification is received by April 17, 2018. Notification may be to FAHQ via email ConferenceRegistration@FAHQ.org
